ENALAPRIME 5 Enalapril Mateate (5mg) ENALAPRIME is an angiotensin-converting enzyme (ACE) inhibitor widely used to treat high blood pressure (hypertension) and heart failure. By inhibiting the enzyme responsible for converting angiotensin I to angiotensin II, enalapril decreases the production of angiotensin II, leading to vasodilation and reduced blood pressure. Additionally, ENALAPRIME helps reduce the workload on the heart and can improve symptoms of heart failure. Adverse Effects: ENALAPRIME can cause several side effects, including hypotension (low blood pressure), hyperkalemia (high potassium levels in the blood), angioedema (swelling under the skin), cholestatic jaundice (jaundice caused by bile flow obstruction), and hypersensitivity reactions. Hypotension is often due to the drug's action of vasodilation, while hyperkalemia results from reduced potassium excretion by the kidneys. Angioedema, though rare, can be serious, particularly if it involves the head, neck, or intestines, potentially compromising the airway or causing abdominal pain. Enalapril can also impact the liver, leading to cholestatic jaundice and, in extreme cases, fulminant hepatic necrosis, necessitating discontinuation of the medication if these occur​​. Contraindications: The use of ENALAPRIME is contraindicated in individuals with a history of angioedema (idiopathic, hereditary, or associated with previous ACE inhibitor use), hypersensitivity reactions to ACE inhibitors, and during pregnancy and lactation. It should be used with caution in patients with conditions such as aortic stenosis, myocardial infarction, stroke, hypertrophic cardiomyopathy, collagen vascular disease, renal artery stenosis, and renal impairment​​. Drug Interactions: ENALAPRIME interacts with numerous drugs. Some enhance its hypotensive effect, such as alfuzosin and loop diuretics, while others can diminish its antihypertensive effect, like amphetamines and methylphenidate. Certain drugs increase the risk of adverse effects, including hyperkalemia when taken with enalapril, such as potassium-sparing diuretics and potassium salts. Furthermore, enalapril can augment the adverse effects of drugs like NSAIDs and lithium​​. Monitoring: Patients taking enalapril should have their renal function, serum potassium, serum creatinine, blood urea nitrogen (BUN), complete blood count, and hepatic transaminases monitored. This is particularly important in elderly patients and those with conditions that could exacerbate the drug's side effects​​. Toxicity: ENALAPRIME toxicity is rare but can occur, as evidenced by historical cases of overdose leading to acute intoxication, which was managed through symptomatic treatment such as fluid resuscitation for hypotension.
